What is it ?
A
Inflammation of the urethral mucosa from a sexual transmitted infection due to gonococcal infection and non gonococcal infection

What is gonococcal infection ?
A
Neisseria gonorrhea

what is non gonococcal infection ?
A
- chlamydia trachomatis
- Mycoplasma genitalium
- trichomonas vaginalis
- gram positive cocci
- HSV 1 ,2
- adenovirus

Risk factors
A
- unprotected sexual intercourse
- multiple sexual partners
- history of STI

Clinical features
A
- Dysuria
- burning or itching of the urethral meatus
- urethral discharge : purulant , bloody , clear
- initial hematuria
- fever , chills , myalgia

How to confirm urethritis ?
A
- urine dipstick : positive leukocyte Eastrase
- pyuria ( more or equal to 5-10 WBC )

How to identify the organism ?
A
- gram stain of urethral swap or discharge
( gram negative diploccoci ) - NAAT for n . Gonorhea or chlamydia

Features of C.trachomatis or M.genitalium
A
Dysuria with urethral discharge
No organism in gram staining of a urethral specimen

Treatment of NGU
A
Single dose azithromycin out doxycycline for 7 days

Treatment for GU
A
Dose IM ceftriaxone

Treatment of T.vaginalis
A
Metronidazole

When do you repeat NAAT ?
A
3-6 months after therapy
