Hyditidorm Mole Flashcards
What are the 2 types of moles ?
- complete
- partial
If the mole turns invasive what will happen ?
1) it will infiltrate the myometrium
2) it will Hematogenic spread lead to metastasis to the brain lungs liver
What are the risk factors for hydatidiform mole ?
History of molar
History of miscarriage
Age less or is 15
Age more or is 35
What are the features of complete mole ?
1) doesn’t contain any fetal parts
2) fertilization of embryo egg with sperm
3) karyotype 46XX , 46 XY
What are the features of partial mole ?
1) Contain fetal parts
2) fertilization of egg contain haploid set of chromosome with 2 sperms
3)karyotype 69xxx,69xxy,69xyy
What are the clinical features of a complete mole ?
1) vaginal bleeding 1st trimester
2) uterus is greater than gestational age
3) pelvic pressure an pain
4) passage of vesicles
What is the clinical features of complete mole due to high B-HCG .
1) preeclampsia
2) hyperemsis gravidarm
3) ovarian theca lutien cysts
4) hyperthyroidism: increase in HCG , the alpha subunits of HCG structurally resemble TSH
What are the symptoms of the partial mole ?
- less severe
- vaginal bleeding
- pelvic tenderness
What is shown in lab studies ?
Increase BHCG
What are the complete mole diagnosis in transvaginal ultrasound?
- theca lutien cells
- snowstorm
- no amniotic fluid
- no heart tones
What are the partial mole diagnosis in transvaginal ultrasound?
- fetal part may be visible
- heart tones
- amniotic fluid presents
- increase placental thickness
When do we use chest x ray ?
Dyspnea
Chest pain
What is the treatment ?
Uterine evacuation by dilation and suction curettage
Monitor B HCG
Methotrexate ( chemo )
