What are the risk factors of postpartum Endometritis?

1) c- section
2) prolonged labor
3) multiple cervical examinations
4) retained products of conception after delivery, miscarriage, abortion

A
- gardenrella vaginalis
- staphylococcus epidermis
- group B streptococcus

Pathophysiology ?
A
Lochia retention leads to
Ideal breeding ground for infection leads to
Postpartum Endometritis leads to
Postpartum sepsis

Clinical features
A
- fever
- lower abdominal pain/ uterine tenderness
- chills , malaise
- foul smelling lochia

Diagnostics?
A
- clinically
- gram stain of vaginal discharge
- blood and uterine culture
- histo: plasma cells in chronic Endometritis

Q
Treatment
A
Antibiotic therapy: IV clindamycin and gentamicin
If patient is clindamycin resistant then ampicillin - sulbactum
- uterine curettage to remove the retained products
- hysterectomy in life threatening cases

Complications
A
- surgical site infection
- pertonitis
- intra abdominal abscess
