Addison’s treatment?
hydrocortisone as cortisol replacement, trying to mimic diurnal rhythm
Fludrocortisone as aldosterone replacement
What should you check in Addisons disease?
TB status
Secondary adrenal insufficiency?
Hydrocortisone as cortisol replacement
Treatment of primary aldosteronism?
If adrenal adenoma, surgery (adrenalectomy)
If bilateral adrenal hyperplasia, use MR antagonists - spironolactone or eplerenon
Phaechromocytoma?
Phenoxybenxbenxamine and atenolol/metoprolol
Surgery (tumour de-bulking)
Chemo
Hypothyroidism?
Give thyroxine (t4)
Hyperthyroidism?
Carbimazole (Propythiouracil in pregnancy)
Beta blockers for symptoms
Thyroid storm?
ABCDE
Iodine, glucocorticoids, PTU, B blockers, fluids, monitoring
Papillary thyroid cancer?
Thyroid lobectomy with isthmusectoctomy, sub total thyroidectomy
Iodine Ablation
Hypercalcaemia
fluids - 0.9% saline
loop diuretics (NOT thiazides)
Bisphosphonates
Steroids if sarcoidosis is cause
Hyperparathyroidism
Only treatment option is surgery, but not always needed
Hypocalcaemia
IV calcium gluconate
Paget’s?
bisphosphonate high dose
Calcitonin
Thyroglossal cysts
surgical removal
Syndrome of inappropriate ADH secretion?
fluid restriction
