Why does a non-haemolytic febrile reaction sometimes occur with blood product transfusions?
antibodies reacting with:
1. white cell fragments in the blood product
2. cytokines that have leaked from the blood cell during storage
How often do non-haemolytic febrile transfusion reactions occur?
Red cell transfusion (1-2%)
Platelet transfusion (10-30%)
Symptoms in a non-haemolytic febrile transfusion reaction
Fever/chills
Management of a non-haemolytic febrile reaction
Slow or stop the transfusion
Paracetamol
Monitor
Describe the symptoms of a minor allergic reaction to blood products
Pruritus, urticaria
Why do minor allergic reactions occur with blood products?
caused by foreign plasma proteins

What causes an acute haemolytic reaction ?
ABO-incompatible blood
=> IgM causes intravscular haemolysis
Symptoms of acute haemolytic reaction
Fever
Abdominal pain
Hypotension
Management of acute haemolytic reaction
Stop transfusion
Confirm identity of patient against blood
send blood for direct Coombs test, repeat typing and cross-matching
Supportive care
fluid resuscitation
Cause of Transfusion Associated Circulatory Overload (TACO)
Excessive rate of transfusion, pre-existing heart failure
Symptoms/Signs of TACO
Pulmonary oedema, hypertension
