How long after chemotherapy does neutropaenic sepsis most commonly occur?
A
most commonly occurs 7-14 days after chemotherapy

Most common causative organism of neutropaenic sepsis in cancer patients
A
coagulase-negative, Gram-positive bacteria (Staphylococcus epidermidis)
due to indwelling lines

What antibiotic prophylaxis is used if we think patients are at risk of neutropaenia following treatment?
A
FLuoroquinolone (ciprofloxacin)

Management of neutropenic sepsis
A
- Start Abx immediately
- do not wait for cultures
- Tazocin
- If still febrile after 48hrs escalate to meropenem +/- vancomycin
- if patient not responding after 4-6 days Ix for fungal infection with HRCT
- role for G-CSF in certain patients
