What causes myelofibrosis
A
myeloproliferative disorder
- hyperplasia of abnormal megakaryocytes
- release of platelet derived growth factor is thought to stimulate fibroblasts
- haematopoiesis in the liver and spleen

Presentation of Myelofibrosis
A
- elderly person
- anaemia
- massive splenomegaly
- hypermetabolic: weight loss, night sweats

FBC and biochemical findings in Myelofibrosis
A
anaemia
high WBC and platelet count early in disease
high urate and LDH (increased cell turnover)
