Muscarinic toxidrome, agents, treatment
Muscarinic toxidrome - acetylcholine toxicity/cholinergic
Agents such as organophosphates
DUMBELS or SLUDGE D - diarrhoea U - urination M - miosis B - bronchorrhea and body fasciculations and bradycardia E - emesis L - lacrimation S - salivation
S - salivation L - lacrimation U - urination D - defecation G - GI cramping E - emesis
Treatment is chlonergic antagonist such as atropine and pralidoxime.
Serotonergic toxidrome
Agitation, autonimic instability auch as tachycardia, neuromuscular acitivity such as clonus. Myoclonus, brief hyperreflexia, higher tones. Diaphoresis.
Anticholinergic syndrome
Anticholinergic syndrome - mad as a hatter, blind as a bat (mydriasis), red as a beet (flushed skin), hot as a hare (dry skin/anhydrosis), dry as a bone, heart races alone. Treatment with physostigmine.
Activated charcoal cannot be used in
Needs to be used within 1-2 hours. Cannot be used for acids, alkali, alcohols, ions/metals like iron overdose
