A
Recurrent VT, FHx of sudden cardiac death, massive LVH>30mm, left vent apical asymmetry, extensive late gad enhancement

A
Type 3 familial dyslipidaemia - dysbetalipoproteinaemia which usually shows cholesterol and TG elevation and causes atherosclerosis

Eruptive vs tendinous xanthoma is indicative of?
A
Eruptive - high TG, tendinous - high cholesterol

Indications of CRTD
A
- Sinus rhythm
- Heart failure despite optimal medical treatment for at least 3 months (lvef<35%)
- QRS > 150ms, can consider in >120ms
- LBBB morphology
- NYHA class 3 or 4

Indications for ICD
A
For ischaemic cardiomyopathy
1. LVEF < 30% with NYHA 1 or LVEF <35% with NYHA 2 or 3
2. >40 days post MI
3. >90 days post revascularisation
Or
Stable VT
Cardiac arrest secondary to VT or VF
Unexplained syncope with inducible VT
