Thromboembolic Venous Disease Flashcards
1
Q
What accounts for the majority of embolic events?
A
Deep veins of the LE above the knee, pelvic and abdominal venous structures
2
Q
What is Virchow’s triad?
A
Vascular injury
Venous stasis
Hypercoaguable state
3
Q
Discuss Ventilation/Perfusion Mismatch
A
- Some areas of the lung is not receiving blood flow leading to Impaired gas exchange.
- Pulmonary edema results from decreased surfactant production by Type 2 pneumocytes
4
Q
How can pressure overload in the RV and PA lead to thromboembolism?
A
- RV pressure overload causes bulging of septum and decreased amount of preload available to LV
- PA pressure overload causes pulmonary hypertension and decreases lung ability to oxygenate
- <33% occlusion needed in pts with comorbid conditions to cause RV strain
5
Q
Symptoms of Thromboembolism
A
Hemoptysis
Pleuritic Chest pain
Dyspnea
6
Q
What test has a perfect negative predictive value for thromboembolic disease?
A
D-Dimer(Fibrin split product)
7
Q
How is thromboembolic disease diagnosed and treated?
A
Spiral ct; Heparin
