In which patients should there be a high degree of suspicion of a TPT?
A
- Chest trauma
- Ventilated pt
- Suspected spontaneous pneumothorax

What are the criteria for a simple pneumothorax? (3)
A
Any of the following:
- Unequal breath sounds in the spontaneously ventilating pt
- SPO2 <92% on RA
- Subcutaneous emphysema

What is the Mx for a simple pneumothorax?
A
- O2 as indicated
- Analgesia as indicated
- Position upright if appropriate
- Monitor for signs of TPT

What are the criteria for a TPT? (8)
A
- increased resp. distress in the awake pt
- decreased SPO2 to <92% despite O2
- decreased CS
- poor perfusion or inc. HR +/- dec. BP
- increased peak inspiratory pressure/stiff bag
- decreased ETCO2
- increased JVP
- tracheal shift
+/- signs of a simple pneumothorax

What are the two Mx arms of the TPT guideline, and how are they both managed?
A
Cardiac arrest imminent
- BP <70, GCS<10 then decompress immediately (ALS)
Cardiac arrest not imminent
- Monitor for deterioration (ALS), decompression (MICA)
