COPD should be suspected in…
Any patient over 40 with:
- Smoking Hx
- Dyspnoea that is progressive, persistent and worse with exercise
- Chronic cough
- Chronic sputum production
- Family Hx COPD
An exacerbation of COPD can be described as?
- Increased dyspnoea
- Increased cough
- Increased sputum production
- Complete removal of wheeze may not be possible due to chronic airway disease
What is the management for all exacerbations of COPD?
Irrespective of severity:
Salbutamol 10mg (5mL)
Ipratropium Bromide 500mcg (2mL)
Dexamethasone 8mg IV/Oral
NO REPEATS OF ANY.
If the pt adequately responds to the initial management, then?
Titrate to SPO2 of 88-92%, consider low flow O2 via nasal prongs.
What is the timeframe for assessing whether a patient has inadequately responded to the initial management?
10 mins.
If the pt inadequately responds, then?
If still severe resp. distress and RR >24 then CPAP required (MICA).
If the pt deteriorates with inadequate VT or RR then…?
Provide assisted ventilation with 100% O2
