Surgical Treatment for CCL Rupture Flashcards
Extracapsular reconstruction
Simulates CrCL
Lateral suture - suture behind fabella pr around bone anchor and make a tunnel through tibial tuberosity
Tightrope CCL - creation of tunnel through femur and tunnel through tibial tuberosity
Tibial plateau leveling osteotomy (TPLO)
Renders CrCL irrelevant
Levels the tibial plateau to prevent tibia moving cranially; utilizes the tibial plateau angle to level plateau into horizontal position
Tibial tuberosity advancement (TTA)
Simulates CrCL
Cut through cranial border of tibia and moving cranially; fix in place with bone graph to allow patella ligament to act as CrCL
Primary ligament
Simulates CrCL
Uses suture in place of of CCL
Intracapsular reconstruction
Simulates CrCL
Uses strip of fascia lata to act as ligament. Fascia lata strip runs through tibial tunnel, through the joint space, through a femoral tunnel or over the top of lateral condyle, and attaches to the distal end of femur
Fibular head advancement
Simulates CrCL
Dislocate tibial-fibular joint and advance cranially so the lateral collateral ligament can act as CrCL
Tibial wedge osteotomy
Renders CrCL irrelevant
Determine TPA and remove wedge shape of the corresponding angle from the tibia, connecting remaining part using bone plate - levels the tibial plateau
