Spondyloarthropathies Flashcards
what is spondyloarthropathies
chronic systemic inflammatory disorder characterised by involvement of both spine and joints (genetically predisposed)
what genetic predisposition is associated with spondyloarthropathies
HLA B27
what other conditions are associated with HLA B27
ankylosing spondylititis
reactive arthritis
crohn’s
uveitits
what what ankylosing spondylitis
form of arthritis mainly affecting the spine
what are the 4 subgroups of spondyloarthritis
ankylosing spondylitis
psoriatic arthritis
reactive arthritis
enteropathic arthritis
what is mechanical back pain like
worsened by activity, typically worse at the end of the day, better with rest
what is inflammatory back pain like
worse with rest
better with activity
significant early morning stiffness (>30mins)
what are some shared rheumatological features of the spondyloarthroapthies
sacroiliac and spinal involvement
Enthesitis (inflammation at the insertion of tendon into bone)
Inflammatory arthritis
dactylics (inflammation of entire digit)
some extra rheumatological features of spondylarthropathies
ocular inflammation
mucocutaneous lesions
rare aortic incompetence or heart blood
no rheumatoid nodules
does ankolosying spondylitis affect men or women more
men
what is ankylosing spondylitis
chronic systemic inflammatory disorder that primarily affects the spine
what are the criteria for diagnosing ankylosing spondylitis
- limited lumbar motion
- lower back pain for 3 months (improved with exercise not relieved by rest)
- reduced chest expansion
- sacroilitis on x ray (bilateral grate 2-4, unilateral 3-4)
what are the A symptoms associated with ankylosing spondylitis
Axial arthritis Anterior uveitis Aortic regurgitation Apical fibrosis Amyloidosis Achilles tendinitis plAntar fasciitis
what does ankylosing spondylitis do to the vertebrae
fusses them together
what is the test for ankylosing spondylitis
schober test
spine should stretch from 15cm to 20 cm
