Aldosterone antagonist

What are the common indications for Spironolactone?
A
- Oedema
- Ascites (due to cirrhosis of the liver, malignancy)
- Nephrotic syndrome
- Oedema in congestive heart failure
- Resistant hypertension (as an adjunct)
- Primary hyperaldosteronism (in those awaiting surgery)

What are the common adverse drug reactions of Spironolactone?
A
- Hyperkalaemia
* Gynaecomastia (in men)

What is the mechanism of action of Spironolactone?
A
It’s an antagonist for aldosterone - so in the kidney, there is down-regulation of ENAC channels in the DCT/CD

What are common drug-drug interactions of Spironolactone?
A
- Drugs that increase potassium levels (need monitoring)
* Pregnancy
