Pre-Retrieval Steps

A
  • Remain calm
  • Stop and Think
  • Do not panic (attempt to fix/shield again)
  • Insure radiation boundaries and/or adjust (0.1mSv/h)
  • No one in roped off area
  • notify all NEW workers on site
  • move to safe area
  • constantly monitor exposure device
  • seek assistance
  • notify RSO and client
  • bring in emergency equipment
  • wearing TLD, DRD, PAD (read and record DRD)

Post-Retrieval Steps

A
  • Lock the exposure device
  • Take a surface dose reading and record
  • Tag problematic equipment + remove from service until inspected and approved for service
  • store and secure for transport
  • drop barriers and signs
  • advise site supervisor
  • record names, serial numbers and readings of all involved
  • prepare an incident report
