How does force closure arise?
contraction of the inner and outer myofascial units that will also stabilise the lumbar spine, pelvis and hip
What is the inner unit?
Pelvis floor, transverse abdominus, multifidis, diaphragm (core/ local stabilisers)
What is the outer unit?
made of 4 different groups of muscles that act together to produce a sling anterior oblique sling posterior oblique sling lateral sling longitudinal stance
anterior oblique sling

right internal
connect with…
right adductors via the adductor abdominal fascia
when this group contract it provides stability and compresses the pelvic girdle resulting in force closure ofthe pubic symphysis
Posterior oblique sling
lat dorsi and gluteus Maximus which interconnect thoracolumabar fascia
Longitudinal stance
deep multifidus muscles attach to the sacrum
deep layer of thoracolumbar fascia
sacrotuberous ligament via the long head of biceps femoris
contraction of deep multifidis muscle will rotate sacrum forward and increase the tension of the ligament surrounding the sacroiliac joints and lock the joints for stability
goes from back of skull to lower limb
Lateral sling
gluteus medius, gluteus minimus, ITB, quadratus lumbuom on other side
failed control of lateral sling will lead to a hip drop
Symptoms of pelvic girdle pain
- decreased endurance for sitting, standing and walking
- pain radiating down the back leg
- form of lower back pain

hypermobility

ipsilateral muscle guarding

repetitive trauma, childbirth, history of trauma
pain down the posterior thigh we episodes of sharp pain
dull ache in fixed posture
Displacement
hypermobile joint overriding the articular prominence or severe trauma to the joint
lowered iliac crest
restricted passive motion
