What is ‘medicine reconciliation’?
A process designed to ensure that all medication a patient is currently taking is correctly documented on admission and at each transfer of care (e.g. admission and discharge from hospital and other care settings)
What are ‘pharmacodynamic drug interactions’?
This occurs when two drugs have additive or antagonistic pharmacological effects
What are ‘pharmacokinetic drug interactions’?
Occur when a drug increases or decreases the amount of another drug available in the body by affecting absorption, distribution, metabolism or excretion of the other drug
What things are required for a legal prescription?
Signed with ink (own name), written in permanent ink and contain address of practitioner and date.
