Leitlinientherapie bei Reumathoider Arthritis

A

Monotherapie
1. MTX (15mg/Woche) + Glucocorticoid Alternativen: LEF, SSZ
4-6 Wochen
2. MTX Optimierung + Glucocorticoid
4-6 Wochen
3. DMARD kombi MTX+LEF; MTX+CsA; MTX+SSZ+HCQ Alternative: MTX + Biologikum (bei hoher RA-Aktivität)
3 Monate
4. 1. Biological: TNF-α Hemmer, Abatacept, Tocilizumab+MTX Alternative: MTX + Anakinra
3 – 6 Monaten
5. Wechsel auf 2. Biological: TNF-α Hemmer, Abatacept, Tocilizumab, Rituximab +MTX
Bei weiterer Therapierefraktären Erkrankung ->
6. Tofacitinib (JAK-Inhibitor), Azathioprin, Ciclosporin A,
Cyclophosphamid, andere immunmodulierende Therapieverfahren
