What is Parkinson’s Disease?
Neurodegenrative disorder resulting from degeneration of dopamine producing neurones in the basal ganglia
What is the role of the basal ganglia?
- Regulate planning, initiation & termination of movement
- Regulates muscle tone
- Controls subconscious & contraction of skeletal muscle
What is the main source of dopamine?
Substance Nigra
Which pathway promotes movement?
Direct
Which pathway inhibits movement?
Indirect
What is dopamines effect on both pathways?
It excites direct & inhibits indirect
So with a lack of dopamine there is a lack of excitation of direct and a lack of inhibition of indirect causing an overall lack of movement
State some clinical features of PD
Slow or no movement
Resting tremor in fingers
Rigidity
Difficulty initiating movement
State some other secondary clinical features
Shuffle Loss of facial expression Monotone speech Micrographia Sleep disorder Problems swallowing Depression/anxiety
State some medication.
Sinemet, madopar act as dopamine replacement
