A deficiency in blood cells of all lineages

Decreased production
Increased destruction

Bone marrow failure

An example of an inherited marrow failure syndrome?
Fanconi’s anaemia
What is the problem in fanconis anaemia?
Unable to correct inter- strand cross links - DNA damage
Median age in fanconis anaemia ?
7 years old
Cells count features in fanconis anaemia?
Macrocytosis followed by thrombocytopenia and neutropenia
Examples of acquired primary bone marrow failure
Idiopathic aplastic anaemia
Myelodysplastic syndromes
Acute leukaemia
What happens in myelodysplastic syndrome?
Increased apoptosis of progenitor and mature cells
What can myelodysplastic syndrome evolution into?
AML

Drugs - chemo, alcohol, chloramphenicol B12/ folate deficiency Storage disease Infection - viral , mycobacterial Metastatic cancer
Cause of increased destruction of cells causing pancytopenia?
Hypersplenism
Causes of hypersplenism?
Splenic congestion - portal hypertension / congestive cardiac failure
Rheumatoid arthritis
Splenic lymphoma
