-ovary and part of fallopian tube is twisted
-causes disruption of blood supply to ovary
-without surgical intervention, ovary becomes ischaemic and necrotic

-lower abdo pain (severe and one side)
-nausea and vomiting
-difficult to diagnose pre-hospitally but important to consider if thinking about not conveying
