OPTEC- Treatment planning Flashcards
Label this smile line?
This is a high smile line- the top of the gums and the teeth are visible.
Label this smile line?
This is a middle smile line.
Label this smile line
This is a low smile line- The gums are not on show which allows the margins to be hidden.
What is the immediate stage of treatment planning?
This is the relief of acute symptoms (e.g. the patient has presented with pain)
Extraction?
Endodontics?
Immediate dentures or bridges?
What is the initial stage of treatment planning?
This is the disease control.
Extraction of hopeless teeth
HPT
OHI &diet advice
endodontics
management of caries (direct/ provisional restorations)
Denture design
Wax ups for fixed prosthodontics.
What is the re-evaluation stage of treatment planning?
This is to see if your disease control has worked.
We:
Reassess the periodontal status
Confirm denture/ bridge design.
What are the 6 principles of crown preparation?
- preservation of tooth structure
- Retention and Resistance
- Structural durability
- Marginal integrity
- Preservation of the periodontium
- aesthetics
Discuss the impact of over and underpreparing the tooth prior to a crown.
Overpreparing a tooth- will cause weakening of the tooth and damage of the pulp.
Underpreparing the tooth- poor aesthetics and it will be overbuilt (this can encroach on the supracrestal attachment causing gingivitis and periodonitis)
Compare the retention and resistance of a crown.
Retention prevents the removal along the path of insertion.
This is done by tapering the tooth at a 6* angle to limit the paths of insertion.
Resistance prevents dislodgement
This is achieved by creating longer walls, so the crown will have to move further before it dislodges.
How do we ensure structural durability of the restoration.
We ensure that the material is thick by:
- Occlusal reduction
- Bevelling the functional cusp (you round off the cusp subjected to the occlusal load- we don’t want to have any sharp edges that could cause a fracture)
- Axial reduction- we want to reduce the tooth in at least 2 planes.
When do we choose a chamfer margin?
Veneers or if the outside restoration is made of metal .
When do we choose a shoulder margin?
If we are preparing for a ceramic crown.
How do we preserve the periodontium?
Margins should be:
- Smooth and fully exposed to a cleansing action
- Placed so that the dentist can finish them and the patient can clean them.
- Placed at the gingival margin (if possible )
