Categories of Gestational Age stressors
Preterm - 42 weeks
LGA - 90th percentile weight
SGA - 10th percentile weight
Levels of nursery care
Level 1 - low risk
Level 2 - moderate-high risk
level 3 - tertiary-high risk - neonatal resuscitation
Common problems with preterm infants
Respiratory problems, thermoregulation, fluid/electrolye, skin, pain, and nutrition.
RDS symptoms
Respiratory depression syndrome Tachypnea, nasal flaring, grunting. Treated: Suction, 02 (hood or cannula), endotrachial tube to insert surfactant ventilator or positive pressure.
BronchoPulmonary Dysplasia
Chronic lung disease caused by mechanical ventilation into lungs. Mechanical ventillation can actually destroy the lungs or cause atelectasis.
Treated: Maintain o2, steroids/diuretics/antibiotics, nutritional supplement, and hydration/suction
Postterm risks
Asphyxia in the uterus, mconium aspiration, interrupted umbilical blood flow, and poor placental perfusion (from it dying)
When to begin cardiac compressions
FHR
Extracorporeal membrane oxygenation
Breathes for the infant and oxygenates blood in meconium aspiration.
LGA and SGA complication
hypoglycemia, especially for LGA.
