NSAID mechanism of action
NSAIDS block “COX” enzymes which in turn inhibits prostaglandin formation to decrease:
COX 1 gastric protection renal perfusion platelet aggregation COX 2 pain fever inflammation
common issues w/NSAIDs
GI issues
renal impairment - watch Is/Os, BUN, and creatinine
heart attack and stroke - elevated risk w/ALL NSAIDS except aspirin
GI issues associated w/NSAIDs
damage to gastric mucosa
sign of bleeding - dark stool, coffee ground emesis, ecchymosis
prevention:
take w/food or milk
take proton pump inhibitor or histamine 2 blocker
aspirin poisoning
"salicylism" tinnitus diaphoresis headache vertigo acid-base disturbances--> increased RR/kussmaul respi
aspirin toxicity tx
activated charcoal w/in 1-2 hours
increase IV fluid to make pt pee
sodium bicarbonate to increase PH of blood
