characterized by sever, enduring disturbances in mood
ranges from extreme elation to extreme depression

anhedonia - lack of enery or interest
lasts at least 2 weeks
is it related to grief? if so it is not MDE
Manic episode
1 week or less exaggerated elation, euphoria no depression is involved, up cognitive symptoms, extreme self esteem, grandiosity depressed need for sleep
Hypomanic episode
not as high of a mood as manic
less severe
at lest 4 days
never hospitalized
Mixed features
not an episode but something to note
feeling both elation and depression at the same time
at least 3 symptoms of another episode (MDE+manic)
Manic depressive disorder (MDD)
overwhelming sadness, despair, hopelessness, inability to experience pleasure/loss of interest and anhedonia
absence of manic and hypomanic episodes
85% of people who have one episode have more
MDD differential diagnosis
not due to medical condition
substance/medication induced
anxiety disorders
Additional criteria for MDD
specify if episode is mild, moderate or severe
is it single or recurrent
specify features of current/most recent episode
-with mixed features
Anxious distress
at least 2 anxiety symptoms during the majority of MDE
With melancholic features
more severe physical symptoms
ex. weight loss, no sex drive, anhedonia, early morning awakening, guilt with no need for it.
With atypical feautres
over sleeping and over eating / weight gain
with psychotic features
delusions and/or hallucinations present
with catatonic features
absence of movement
muscles and body parts semi rigid
rare
with seasonal pattern
in recurrent MDD , episodes only occur at certain times of the year
seasonal affective disorder
2+ years of seasonal MDEs w/out any MDEs during any other time
Persistent Depressive Disorder (dysthymia) (PDD)
Persistent depressed mood w/ low self esteem, withdrawal, pessimism, and/or despair for 2 years
can not be w/our symptoms for more than 2 months typical early onset adolescence
early onset is before 21 w/ greater chronicity
stronger likelihood of family history
may last decades
Double Depression
MDD+PDD
specify mild, moderate, severe or if there are mixed features
PDD occurs first
Specific to PDD
w/ pure dysthymic symdrome
full criteria for MDE has not been met in 2 years
w/ persistent MDE, full criteria for MDEs have been met
w/ intermittent MDEs w/ current episode
