β Blocker

Mechanism of Action:
β1 R-antagonist ⇒ negative inotropy, chronotropy
METOPROLOL (Lopressor®)
Pharmacokinetics
Onset of Action = Immediate
Peak Effects = 20 minutes
Duration of Effect = 13-19 hours (or 5-8hr?)
Half-Life = 3-4 hours

Indications/Field Use:
- SVT refractory to adenosine
- A-fib, a-flutter, stable narrow complex tachycardia
METOPROLOL (Lopressor®)
Contraindications:
- HR < 45 BPM, Systolic BP < 100mmHg
- CHF
- any heart block
- asthma
- shock
- bronchospasm
METOPROLOL (Lopressor®)
Side Effects/Adverse Reactions:
- Bradycardia
- hypotension
- lethargy
- CHF
- dyspnea
- wheezing
- weakness

ADULT DOSE:
(ADULT): 5mg slow IVP (IO) q 2-5min, max dose 15mg

PEDI DOSE:
Not for Pediatric Patients
METOPROLOL (Lopressor®)
Precautions:
- Watch for onset CHF
- shock
- bronchospasm
- bradycardia
- heart block
METOPROLOL (Lopressor®)
Drug-Drug Interactions:
- Caution Pt.’s taking antihypertensives
- do not give to Pt. who has received IV Verapamil (or other Ca2+ channel blocker?)
