Metoclopramide Flashcards

1
Q

What are the indications for administering

metoclopramide?

A

It is administered to stimulate gastric emptying. It functions as
an antiemetic, as a treatment for gastroparesis, and can assist
in the placement of feeding tubes.

Q

What local anesthetic is metoclopramide structurally

similar to?

A

Metoclopramide is structurally similar to the ester local
anesthetic procaine and the benzamide antiarrhythmic
procainamide.

Does it cross the placenta?

A

Yes, metoclopramide crosses the blood-brain barrier and the
placenta easily

Q

In addition to dopamine, what other receptor can

metoclopramide antagonize?

A

In high doses, metoclopramide can also block serotonin
receptors in the chemoreceptor trigger zone

Q

What drugs increase the likelihood of extrapyramidal

symptoms in a patient taking metoclopramide?

A

When administered concurrently with metoclopramide,
phenothiazine derivatives such as thioridazine and
chlorpromazine and butyrophenones (droperidol) increase the
likelihood that the patient will experience extrapyramidal
symptoms.

Q

What are the most common side effects of

metoclopramide?

A

Metoclopramide can produce CNS side effects such as
restlessness, drowsiness, and agitation (in 10-20% of the
population). 25% of patients who are administered
metoclopramide in high doses or over a long period of time will
develop extrapyramidal symptoms such as dystonias, akathisias
and tardive dyskinesia.

Q

Is metoclopramide contraindicated in obstetric

patients?

A

Although it crosses the placenta easily, it is not contraindicated
in obstetric patients undergoing surgery.

Q

How does metoclopramide affect the muscle tissue

of the stomach?

A

It increases the tension of the lower esophageal sphincter and
fundus, while relaxing the pylorus and duodenum.

Q

How is intravenous metoclopramide metabolized?

Orally-administered metoclopramide?

A

If administered intravenously, about 70% of metoclopramide is
conjugated in the liver. If administered orally, about 75% is
quickly eliminated by hepatic first-pass metabolism.

and those who have had gastric surgery.

Q

How does metoclopramide affect the action of

succinylcholine?

A

the duration of succinylcholine.
