What is Meniere’s disease?
Auditory disease characterised by an episodic sudden onset of vertigo, deafness and tinnitus
What is the pathophysiology of Meniere’s?
Over-production or impaired absorption of endolymph - causes distension/rupture of Reissner’s membrane
What are risk factors for Meniere’s?
Recent viral infection
Autoimmune condition
What are the symptoms of Meniere’s?
Vertigo episodes
Unilateral hearing loss
Tinnitus
Aural fullness
What examination tests can be done?
Romberg’s test - postitve
Fukuda’s stepping test
What is Romberg’s test?
Swaying or falling when asked to stand with feet together and eyes closed
What is Fukuda’s stepping test?
Turning towards the affected when asked to march in place with eyes closed
What does audiometry show in Meniere’s?
Unilateral sensorineural hearing loss - usually low frequency
What are differentials for Meniere’s?
Acoustic neuroma
Vestibular migraine
Viral labyrinthitis
BPPV
How is Meniere’s managed?
Refer to ENT or neurology
Low salt diet + diuretics
- vertigo
Betahistine
What advice is given to patients with Meniere’s?
Restrict salt intake Limit caffeine intake Reduce alcohol consumption Smoking cessation Manage stress
Chronic disease - may wax or wane over time
What is the follow up for Meniere’s?
Regular hearing tests
