If a pt inquires about MAID, what are the two options for the clinician?
Conscientious Objection
Proceed with MAID Discussion
What are the aspects of the MAID discussion that must be included in addition to information about MAID?
Exploration of patient’s suffering, psychosocial or non-medical conditions and circumstances
Alleviate concerns that may impact the request such as psychosocial factors or lack of resources
What are the next steps in management if a physician has a conscientious objection?
Communication the objection directly and in a non judgemental manor.
Inform the pt the objection is personal and not medical.
Provide information about all options
A referral must be made
If a pt opts to proceed, what are the criteria for MAID:
Be eligible for publicly funded health care
Be at least 18 y/o and capable of making a decision w/ respect to their health (i.e. have capacity)
Have a grevous and irremediable medical condition (illness, disease, disability, NOT mental illness) that is in an advanced state of irreversible decline, and the patient is enduring physical or psychological suffering that is intolerable to them and cannot be relieved under conditions that are considered acceptable.
Make a voluntary request for MAID that is NOT the result of external pressure
Provide informed consent to receive MAID after having been informed of the means that are available to relieve their symptoms including medical diagnosis, available forms of treatment, available options to relieve suffering including palliative care
Consent must be given at the time of request and immediately before MAID unless special circumstances apply
If the patient is assessed and meets the criterial for MAID, what are the next steps?
Request must be made in writing and signed by 1 independent witness
Two independent clinicians must assess eligibility
Pt is informed of ability to withdraw request
What occurs if Natural Death is NOT foreseeable?
Expert consultation assessing the status and treatment options to reduce suffering with a written report to both independent clinicians
90 day waiting period
What are the two options for MAID if Natural Death is Forseeable?
Clinician Administered MAID
Self Administered MAID
If the pt chooses clinician administered MAID what is needed immediately prior to administration of MAID? What if the patient is at risk of losing decision making capacity prior?
Consent is required immediately prior to administration
Can provided a waiver of final consent if patient is at risk of losing decision making capacity.
If the pt chooses Self Administered MAID, what can be done if self administered MAID fails?
Advanced consent for failed self administration – allows clinician to administer MAID is there is a complication
