lipoglycopeptide Flashcards
what are these drugs?
vancomycin analogues
what are the agents
dalbavancin, telavacin, oritavancin
what is unique about these
they have a lipophilic side chain that increases binding
what general mechanism to there all share?
they all inhibit the transglycosylation by binding to the d-ala-d-ala, just like vancomycin
what additional mechanisms does oritavancin have
interacts and disrupts the cell membrane. binds the pentagly bridging segment and inhibits the transpeptidation
what additional mechanism does telavancin have
interaction and disruption of the cell membrane.
what isi the spectrum of dalbavancin
(+): MRSA, MSSA, strep, coagulase (-) staph. anaerobes: clostridium species, p acnes, peptostreptococcus.
what is the spectrum for oritavancin
same as dalbavancin but active against VRE.
what is the spectrum for telavancin
same but also active against VRE.
what are the relative half lives of these drugs
telavancin has the least do to renal excretion. dalbavancin has the highest at 257hrs.
what is the clinical use of these drugs?
skin and soft tissue infections.
what are the SE/
metallic taste, nausea, HA, nephrotoxic. teratogenic.
