binds to the D-ala-D-ala terminus of the nascent peptidoglycan pentapeptide. inhibits transglycosylase preventing the elongation of the peptidoglycan and cross-linking

gram (+): MRSA, MSSA (less sensitive betas), enterococcus, coagulase (-) staph (DOC), strep.

target modification (enterococcus), trapping in the cell wall (staph makes a thicker wall when mutated), VRSA uses a vector to and uses target modification.
what are the clinical uses for vancomycin
MRSA (skin, soft tissue, pneumonia, meningitis). C. diff, coagulase negative staph, MSSA with penicillin allergy
what are the adverse affects from vanco
renal dysfunction, HSR (rash, hives), red mans syndrome
what is red mans syndrome
histamine mediated (not allergic). stopped by slowing infusion
what is the downfall of vanco?
bulky and poorly penetrates the BBB and lungs. but still effective.
