After a Qasm (oath) the affirmative maadi should be emphasised with لَقَدْ - the negative maadi needs no emphasis.
Also note the و of qasm is H-J

The verb ماتَ comes in two forms
a) i-a ماتَ يماتُ (in the mutaharrik cases the first radical takes kasra in maadhi - مِتُّ) - in Q 9x
b) a-u and ماتَ يموتُ (in the mutaharrik cases the first radical takes dhama in the madhi مُتُّ) - in Q 2x
In mudhaari only يموتُ is in Q.
