What is the highest risk to mother due to a large baby?
Shoulder dystocia - where baby’s shoulder gets stuck behind pubic bone
What is the medical word for baby that is large for gestational age?
Macrosomia
What must you rule out/check on USS?
EFW
Rule out polyhydramnios
What are the USS parameters for polyhydramnios?
AFI (amniotic fluid index) >25
DVP (deepest vertical pocket) >8cm
What is polyhydramnios and what are the risk factors?
Too much amniotic fluid in the amniotic sac
Most cases are idiopathic
Maternal diabetes is a risk factor
Most cases of polyhydramnios is asymptomatic but it can present with abdo discomfort.
Name 2 signs which may be seen on examination
Tight and shiny abdo
Loss of ability to feel foetal parts - due to excess fluid
How is polyhydramnios managed?
Induced labour @40wks
Serial USS
Most cases monitored just be watching
What constitutes macrosomia?
> 4.5kg at birth
EFW >90th centile
AC >97th centile
What risks are there during labour of macrosomic baby?
PPH - post partum haemorraghe
Shoulder dystocia
Labour dystocia - inability to proceed through labour
How much amniotic fluid is thought to be present in polyhydraminos?
> 2-3l
