What symptom is endometrial cancer until proven otherwise?
Post menopausal bleeding
What are the two big risk factors?
- Obesity
- T2 Diabetes
Explain how the following conditions increase your risk:
- PCOS
- Obesity
PCOS
- less likely to ovulate -> reduced number of corpus lutems -> reduced progesterone production -> more unopposed oestrogen exposure
Obesity
- source of oestrogen -> adipose cells contain AROMATASE -> Aromatase converts testosterone into oestrogen
What hormone fuels endometrial cancer?
Oestrogen - causes hyperplasia of endometrial lining
What kind of cancer makes up 80% of endometrial?
Adenocarcinoma
What 4 things act as protective factors?
- Smoking
- Mirena coil
- COCP
- Increased no. of pregnancies
Explain what would be cause for an urgent (2 week referral) and what would cause a routine referral?
What would come as a result of a routine referral?
Urgent - post-menopausal bleeding
Routine - >55 w/ unexplained vaginal discharge or visible haematuria
Routine = TVUS
What is a normal post-menopausal endometrial thickness?
<3mm (in Scotland can be <4mm in England)
What is the common age range?
50-60
What kind of abnormal bleeding may patients present with?
- Heavy
- Intermenstrual
- Post-coital
What are the 4 subtypes of endometrial cancer?
Endometroid - 80%
Serous
Much less common:
- Mucoid
- Clear Cell
What sign would make endometrial hyperplasia be termed atypical?
“Rounding-up” - round nuclei in cells surrounding glands
What is the only way to be able to differentiate between ends. hyperplasia and cancer?
