Correct breastfeeding position:
It is important for mother and baby to be belly to belly in order for the infant to be in a good position to latch on appropriately, taking a large part of the areola in its mouth
What bacteria is mastitis usually caused by?
Staphylococcus from the babys mouth
How is mastitis treated?
Antibiotics(usually sensitive to penicillin or cephalosporin); if infection persists abscess may develop and incision and drainage may be necessary
What is the physiology of milk production?
with delivery, there is a rapid decrease in progesterone and estrogen, which removes inhibitory influence of progesterone on production of alpha-lactalbumin by the rough ER. Alpha-lactalbumin stimulates lactose synthase to increase milk lactose. This may take up to 2 days.
Which organism causes tips of the nipples that are pink and shiny with peeling at the periphery after breastfeeding?
Candidiasis, which should prompt inspection of baby’s oral cavity; it is associated with severe discomfort and pain
How can localized candida of the nipple be treated?
antifungal topical medication such as clotrimazole or miconazole cream and may also include topical antibiotic ointment because nipple fissures can concurrently present with candidiasis of the nipples and S. aureus is associated with nipple fissures, either a triple antibiotic ointment or mupirocin can be prescribed; topical steroids for cases in which nipples are very red and inflamed; baby treated with oral nystatin followed by oral fluconazole.
Depot medroxyprogesterone is known to cause ___.

What are the signs that a baby is getting sufficient milk?
3-4 stools in 24 hours, 6 wet diapers in 24 hours, weight gain, and sounds of swallowing
What should a pt do if they have breast engorgement?
frequent nursing, taking a warm shower or warm compresses to enhance milk flow, massaging breast and hand expressing some milk to soften breast, wearing good support bra, using analgesic 20 mins before breastfeeding
More frequent suckling will stimulate which hormone?
Oxytocin, which is responsible for milk ejection and suckling works better than a breast pump. Cortisol and insulin act in concert with other hormones to stimulate growth and development of milk-secreting apparatus. Prolactin is responsible for milk production.
