What are fibrocystic breast changes?
most common type of benign breast condition and occur most during reproductive years; fibrocystic disease is often associated with cyclic mastalgia, possibly related to pronounced hormonal response; caffeine intake can increase the pain
If FNA is done on breast mass and returns clear fluid and mass resolves, what do you do for followup?
reexamine in 2 months to see if cyst resolved
What do you do if breast mass FNA produces bloody fluid in addition to mammogram?
excisional biopsy; normal mammogram does not rule out breast cancer, especially in setting of bloody dishcarge
When does lactational mastitis most often occur and how does it present?
during first 3 months of breastfeeding; firm, red, painful area of one breast often associated with fever
how do you manage lactational mastitis?
ibuprofen and cold compresses for pain relief, continue breastfeeding, oral antibiotics(most often caused by S. aureus)
what can a breast ultrasound do?
distinguish between a cyst and a solid mass
When is breast MRI indicated in addition to mammography?
may be indicated in patients with increased risk of breast cancer: BRCA mutation carriers, first degree relatives of BRCA carriers, genetic mutations such as Li Fraumeni, women with 20-25% risk of lifetime breast cancer, or women with history of radiation to chest between 10 and 30 years old
