A

due to pre-existing antibodies against ABO or HLA antigens
type II hypersensitivity reaction
graft must be removed

usually due to mismatched HLA
cell mediated
asymptomatic- rising creatinine, pyuria and proteinuria
may be reversible with steroids and immunosuppression
