Non physiologic jaundice

A
onset before 24 hours 
rise > 8.5 mmol/l/h 
signs of underlying illness - vomiting, lethargy, poor feeding, excessive weight loss, apnoea, tachypnoea, temp instability 
> 8d term, 14d premature 
direct bilirubin > 34 mmol/l at any time

Prolonged Cholestatic jaundice

> 14 days

A

Conjugated fraction greater 20% total bilirubin
hepatobiliary dysfunction
obstructive - biliary atresia, choledochal cyst, CF
hepatocellular - viral CMV, HIV, UTI, sepsis
genetic/metabolic - a-antitrypsin deficiency, tyrosinaemia, galactosaemia, hypothyroidism, CF
toxic, secondary - TPN, drugs, toxins, shock, hypo perfusion
