What is Irritable bowel syndrome (IBS)?
Chronic disease (>3months) characterized by abdominal pain or discomfort that occurs in association with altered bowel habits
What is believed to play a large role in IBS?
Mental health
What age does IBS normally begin?
Late teens to early twenties
Hallmark sx of IBS?
Abdominal discomfort relieved after defecation
What kind of abdominal pain do patients with IBS have?
Intermittent, crampy in lower abdominal region
Dx of IBS is based on what 2 out of 3 features?
Relieved with defecation
Onsite associated with a change in frequency of stool
Onsite associated with a change in form of stool
What 3 categories are patients with IBS classified?
IBS with constipation (IBS-C) or/with diarrhea (IBS-D)
Infrequent bowel movements (IBS-U)
IBS with mixed constipation and diarrhea (IBS-M)
What will you find on physical exam of a patient with IBS?
Normal exam
IBS is a dx of exclusion, which means?
Provider should r/o other disorders before considering dx
When should labs be done for IBS?
Not required unless suspicious of other dx
What imaging should be considered in patients with IBS?
Young patients: colonoscopy not recommended w/o alarming sx
50+: colonoscopy should be obtained to r/o malignancy
What diets may patients with IBS benefit from?
FODMAPs, low in fermentable oligo-, di-and monosaccharides and polyols
Lactose and gluten avoidance as well
What medications can be given for IBS-D?
Loperamide
What can be given for IBS-C?
Osmotic laxatives
