What is an anorectal abscess?
Obstruction of an anal gland that opens in the base of an anal crypt which normally drains into the anal canal
Anorectal disorder range from what to what?
Simple to complex and varied to multiple
Where are anorectal abscesses frequently encountered?
Perianal and perirectal region
What spaces in the anorectal area can become infected alone or in combination with each other?
Perianal , intersphincteric, ischiorectal, deep postanal, supralevator or pelvirectal space
Where is the most common and least common spaces for anorectal abscesses?
Most common= Perianal
Least common= Supraelvator
What population are abscesses most common?
Young- middle aged males
If a patient notices a dull, aching or throbbing pain that worsens immediately before defecation and is lessened after defecation but persists between bowel movements, what condition might they be experiencing?
Anorectal abscess
Why would a patient experience pain before defecation when they have an anorectal abscess?
Increased pressure in the rectum
What sign and sx may be accompanied with a perianal abscess?
Easily palpable, leukocytosis and sepsis in immunocompromised patients
Other anorectal abscesses (ishiorectal,intersphincteric, supralevator) may present with what sx?
Pt looks uncomfortable, may be febrile and have leukocytosis
Where should perirectal abscesses be drained?
Operating Room
Isolated, simple, fluctuant perianal abscesses may be drained where?
ED or outpatient setting
What gauge needle and incision should be used for draining an abscess?
30g needle, elliptical incision
Is packing necessary for an abscess?
No, but if used, should be removed in 24 hours
How often should sitz baths be done after drainage of an abscess?
3x per day
