What is gastritis?
Histologic evidence of inflammation of epithelial or endothelial of the stomach
What is gastropathy?
Epithelial or endothelial damage w/o histologic evidence of inflammation
What are the 3 categories of gastritis?
Erosive and hemorrhagic gastritis
Non erosive and non specific gastritis
Specific types of gastritis
What are can cause gastritis?
Drugs, alcohol, stress from medical or surgical illness, portal HTN
Less commonly ingestion and radiation
Gastritis is normally, asymptomatic, however what sx can occur?
Epigastric pain, N/V, upper GI bleeding w/ “coffee grounds” emesis
What lab findings are expected with gastritis?
HCT low if significant bleeding
Iron deficiency
H. pylori testing
What is the imaging study is most sensitive method of dx for gastritis?
Endoscopy within 24hrs of upper GI bleed
How would you treat NSAIDS gastritis?
D/C NSAIDS
PPI 2-4 weeks (omeprazole 20-40mg)
How would you treat alcoholic gastritis?
D/C alcohol use
H2 antagonists, PPI for 2-4weeks
What are non erosive, non specific (histologic) types of gastritis?
H pylori
Pernicious anemia
Eosinophilic gastritis
How would you dx H pylori?
Endoscopy
Serology
**Serology not helpful with previous positive anti-body testing
