Midline
incision though the línea alba
poorly vascularised and major nerves are avoided
requires access to the abdominal cavity
Paramedian incision
performed lateral to the línea alba
access to more lateral structures (kidney, spleen and adrenals)
but method legates blood and nerve supply to the muscles medial to the incision
Kocher incision
begins inferior to the xiphiod process and extends inferolaterally in parallel to the right costal margin.
gain access for the gallbladder and or the biliary tree pathology
Chevron incision
The extension of the kocher incision to the other side of the abdomen.
used for oesophagectomy, gastrectomy, bilateral adrenalectomy, hepatic resection and or liver transplant
Mercedes Benz incision
vertical incision and break through the xiphisternum
This may be used for the same indications as the Chevron incision however it is classically seen in liver transplantation.
McBurney’s incision
grid iron= two perpendicular lines
splitting the fibres of the muscles without cutting them good healing
performed at Mc Burney’s point (ASIS and umbilicus 1/3 of distance)
open appendectomy.
