Hip Conditions Flashcards
Iliospoas strain- ss/px/pom
ss- pain that is experienced on the ant hip near groin that typically occurs after a fall/ sedentary jobs
Px- resisted hip flex painful, TT at the iliospoas tendon junction, may present w ant pelvic tilt + hyperlordosis, Thomas test +ve
POM- conservative care
piriformis syndrome- ss/px/pom
ss-radicular symptoms down leg, may walk with toes pointed outward
px- palpation of the piriformis will reproduce neuro findings. Neuro WNL, FAIR, FADDIR, Hibbs +ve
IT band contracture
ss- worse with running uphill, up/down stairs, can hurt on lat hip or knee
px- Renne, Nobal compression +ve, TT TFL, obers pot +ve
Hamstring strain
SS- pain that is felt in deep gluteal area and down back of thigh
px- Pain w resisted knee flex, TT at ischial tub and along muscle belly, Neuro WNL
Hip jt fixation
s/s- inner groin pain that is typically the most sore in morn, typically first ss of OA
px- may present with coxalgia or windmill gait, resisted + pain int rot, FAI +ve, Log roll +ve, Hip scour +ve, FABER, anvil
Quad strain- ss, px, POM
ss- pain on the ant hip
px- pain w resisted knee extension, TT quads from organ to insertion
Adductor strain- ss, px, POM
ss- deep groin pain following pivoting activities, better w rest, tender to palp over damaged tissue, AROM pain during adduction
px- pain w resisted hip adduction
Snapping Hip syndrome
ss- Internal- iliospoas over femoral head, repeatable audible click,
external- IT band glute med/max/TFL snapping over greater trochanter
POM- conserv tx with an emphasis on stretching the tt mm and strengthening tendon that is snapping
Inguinal hernia
ss- when part of the intestine protrudes thru weak spot in the abdominal musculature(scrotum), bulge on either side of pubic bone
px- coughing and crunch testing will cause the hernia to budge out
Intercalated vs strangulated hernia
intercalated- no longer reducible
strangulated- occurs when the vascular supply is compromised secondary to incarceration of hernia contents (emergency)
Meralgia Paresthetica
ss- condition characterized by tingling, numbness, or burning pain in outer thigh due to compression of the lat femoral cutaneous nerve L2-3
px- sustained pressure just medial to ASIS will reproduce thigh pain, Neuro WNL
POM- Conserve, weight management, looser clothing
Osteitis Pubis
ss- Benign inflammation of the pubic symphysis, sclerotic findings seen on pubic bone xray
px-excrutiating pain on palp of thigh adduction and trunk flex
Pom- conservative chiropractic treatment
Congenital hip dysplasia- tests + triad
ss- child complains of limp, groin/hip pain, stiffness in hip joint
px:
Puttis triad (shallow acetabulum, small epiphysis, superolat femoral head)
-Ortalani (distraction w abduction causes relocation)
-Barlow (compression with adduction causes dislocation)
myositis ossificans- ss, px, pom
ss- In quads usually after severe blunt trauma, tender/pain w activation
Px- pain w active knee extension, TT along mm belly
POM- self limiting, limit exercise and movement, contraindication to massage
Avascular necrosis of the femoral head- what it called in adults/children
Chandlers (in adults)
Legg calve perthes (children)
ss- limping, pain/stiffness in the hip, limited ROM, pain worsens w activity
px- Abnormal and painful ROM in younger individual, Antalgic gain, Scour, FABER, Anvil +ve
