- Infection: viral, bacterial fungal, parasitic
- Congestive cardiac failure
- Infiltration: primary and secondary (e.g. neuroblastoma) tumours
- Storage: Fat - cystic fibrosis, glycogen storage diseases
- Idiopathic

- Infection: malaria
- Haematological: hereditary spherocytosis, sickle cell (reduces over time).
- Extramedullary haemopoesis: thallassaemia
- Portal Hypertension
- Neoplastic
