Q

PND mechanism?

A

fluid settles across large SA of lungs = breathless

fluid sinks to bases

Secondly during sleep the respiratory centre becomes less responsive

s= congestion and hypoxia

less adrenalin = myocardium is more relaxed = reduced CO

16
Q

Acute LVF

A

SOB acutely
type 1 respiratory failure low 02 without an increased CO2

cough with frothy white / pink sputum

17
Q

Signs on examination

A

Raised respiratory rate

Reduced oxygen saturations

Tachycardia (fast heart rate)

3rd heart sound

Bilateral basal crackles (sounding “wet”) on auscultation of the lungs

Hypotension in severe cases cardiogenic shock

18
Q

mx of acute LVF?

A

A-E approach
ECG
bloods > anaemia, infection markers, renal screen, BNP consider troponin

ABg
chest XRay
echo

19
Q

what is b type natriuretic peptide?

A

hormone released from the ventricles when the cardiac muscle is stretched beyond the normal range
