Patient Based
Subjective Data (ex. Pain)
Nurse - Observe
Objective Data
A physical examination is a head to toe review of each body system that offers objective information about the client
Health Assessment - 2
What are the different techniques and skills
Inspection, Palpation, Percussion, Auscultation, Olfaction
It is the systematic visual examination of the client
involves observations of the color, shape , size, symmetry, position and movements
Begins with the initial contact with the client and continuous through the entire assessment
Principles
Make sure of adequate light
Pay attention to detail. Do not hurry inspection
Compare each area inspected with the same area on the opposite side of the body
Inspection (eyes - observation)
Involves the use of hands to touch body parts to make sensitive assessments
assessment technique with the use of touch
Palpate the skin for temperature, moisture, texture, turgor, tenderness and thickness
nurses hands should be warm and fingernails should be short and touch should be gentle and respectful
Palpation (hand)
“Pain on touch”
Tenderness
_______ surfaces of the hand and fingers are used to measure temperature, and the ____ surface of the fingers and finger pads are used to assess texture, shape, size, consistency, and pulsation
dorsum (black), palmary (front)
Place the hand on the part you will examine and depress about 1cm (1/2 inch)
Light Palpation
To examine the condition of organs such as those in abdomen
Depress the area approximately 4cm (2 inches)
Apply deep palpation with one hand or both hands
Deep Palpation
