Should vaginal swabs be taken for vaginal problems in children?
They should generally not be performed and if the problem persists then a referal to a gynaecologist is appropriate.
Most newborn girls have some mucoid white vaginal discharge. At what age does this usually disappear?
3 months of age.
What is the most common gynaecological disorder in girls?
Vulvovaginitis
What are some risk factors for vulvovaginitis?
- Poor hygiene (most common)
- Tight clothing
- Infection
Why are young girls prone to getting vulvovaginitis?
- Lack of labial fat pads protecting vaginal orifice
- Lack of protective acid secretions found in the reproductive years
[BLANK] can also present as vulvovaginitis.
Sexual abuse
How do you manage vulvovaginitis?
- Advise about hygiene
- Soothing creams may be useful (nappy rash creams like Bepanthen)
- Topical antibiotics/antifungals
- Sedating antihitamines
- Oestrogen cream in resistant cases
What are some important questions to ask as part of your history?
- Duration of symptoms
- Recent use of medications (e.g. antibiotics), perfumes and bubble bath
- Amount and color of discharge
- Any suspicion of abuse
- Clothing worn
- Any recent respiratory infections
What should you do as part of your examination?
- During the initial evaluation, an examination of the external genitalia with the child in a supine frog-legged position
- Examine the pharynx for evidence of past or current infection
- Closely examine the skin for any rashes or other skin abnormalities
- Check for any signs of sexual abuse
What infections can present as vulvovaginitis?
- Candida infection- associated with antibiotic use.
- Bacteria - E.Coli most common. Steptococcal pyogenes associated with throat infections, Staph Aureus.
- Pinworms, scabies, lice
- Shigella
- STIs
