What is the purpose of the Industrial Injuries GO?
To clarify the responsibilities of employees and supervisors when an employee suffers an injury on the job.
Whenever an industrial injury occurs, whose responsibility is it to fill out all appropriate paperwork?
Each employee and their supervisor.
How is an industrial injury defined?
As an injury or illness arising out of and in the course of COG employment.
Who must be notified of all injuries occurring on-shift?
The immediate supervisor.
Who shall the immediate supervisor notify when an injury is requiring medical treatment?
The safety division officer.
When is the safety division officer available?
On a 24 hour basis.
Who shall the safety division officer notify?
The Fire Chief, Assistant Chiefs and Operations Chief immediately.
If an injured employee requires immediate hospitalization, where shall they be transported?
The nearest, appropriate emergency department.
If the employee’s injury involves a hazardous materials exposure or poison, where should they be transported?
The closest toxicological emergency department. (Banner Good Samaritan)
Burn injuries requiring emergency medical treatment shall be transported to:
The Maricopa Medical Center Burn Unit
Who will be contacted for non-emergency injuries?
The COG occupational health nurse (City Nurse)
After 4 pm or on weekends, what facilities may the injured employee go to?
Banner Thunderbird Occupational Health Clinic, Banner Estrella Clinic, Banner Good Samaritan Clinic
If you sought any type of medical treatment at an outside facility, what must be done?
A follow-up phone call to the city nurse is required.
Whether or not medical treatment is needed, the employee shall immediately report all work related injuries t their immediate supervisor no later than:
No later than the end of the shift.
What must the employee do if the injury becomes apparent after the shift is completed?
Phone a report to the city nurse and complete an employee injury form at first available opportunity.
Who must the employee contact regardless of seeking medical treatment or not?
The city Nurse
When will a physician automatically become the employee’s attending physician?
If the employee has more than one visit to the same physician.
What must the employee obtain for every visit for treatment and whom must they give it to?
They must obtain a written work status and it shall be turned into the city nurse.
Who shall the employee contact if they are unable to obtain a written work status?
The division officer or city nurse for assistance.
Who shall the employee contact if they are placed on a no work status or a modified duty status?
The city division officer and city nurse during regular admin work hours as soon as possible.
The immediate supervisor’s first priority should always be_________?
To ensure that the employee is receiving the appropriate medical care. Employee care is first, documentation is second.
Who shall the immediate supervisor notify if the employee is transported to an emergency room for any reason?
The safety division officer and the on-duty shift commander.
What form shall be completed by the safety division officer?
Industrial paperwork and a hard copy of the form P19.
