Blasto Mode of infection
Inhalation as a pro gating yeast. Blood spread. Does not spread via respiratory as the yeast gets to big. Rare to be a focal skin lesion.
Blasto signalment
Males 1-5 years (2 yrs)
Larger breed and sporting breeds
Blasto signs
Skin eyes brain bone lymph nose; lung; external nares
Blasto testing blood work abnormalities
Hypercalcemia hyperglobulinnemia, anemia mild, leukocytosis
Blasto diagnosis
Gold standard - find organism any stain okay.
LN aspirant and draining lesion > 50%
AGID Blasto
41-90 sensitivity
90-100 specitificty measures antibodies
Ag EIA blasto
93.5 known will be positive. Antigen ursine or serum can do CSF
Amphotericin b mechanism of action
Lipholic; also antibiotic. Binds to ergosterol. Forms a pore tha t allows electrolytes
Poorly absorbed across skin and gut
Ampi b toxicity
Nephrotoxic.
To avoid improve renal blood flow
Lipid base have less toxicity but are less absorbed. Good for CNS disease
Dosing of ampi B
Give every other day in cats and dogs
Azole moa
Block ergostol synthesis and inhibits cell wall formation
Itraconazole
Does not get csf
More selective for fungal. Preferred med
5 mg/kg/day 74% response
Preferred name brand. Not excreted in the urine
Recurrence rate for blasto
20%
Ketocinazole
Absorption better go tract. Does not penetrate cns, ocular, seminal
